to Install Application and Instructions   

The Permit to Install Application Form is now available as a Microsoft Word template. The template allows you to enter information without affecting the form itself. The template may be downloaded and completed on your computer, however the department is not accepting electronic submittal of the application at this time. To complete the form, press the tab or arrow key to move from field to field. Enter data by typing in the gray-shaded fields. When the form is complete, save and print it. You must create three paper copies of the form. The three copies of the application along with two copies of any plans, specifications or drawings required by the instructions should be submitted. The application must include the original signature of an authorized employee of the applicant. This form may be viewed in Adobe Acrobat format.

The Application Instructions provide information available on the back page of the paper application form. The instructions identify the additional information required with the application, and provide the mailing address for submittal of the application. These instructions may be downloaded or viewed in Adobe Acrobat format.

The Information Required for an Administratively Complete Permit to Install Application document clarifies the informational requirements for an administratively complete application for a permit to install. Attached to this document, in Appendix A, is a Checklist for Submitting an Administratively Complete Permit to Install Application. These documents may be downloaded or viewed in Adobe Acrobat format.
