How to perform a Search:
Fill in the fields on the left side of this form with the relevant information for your search. Your search will return all of the records that match the information that you have entered. The fewer fields you fill, the larger the search results will be. If you know the log number, enter it in the appropriate field. A valid log number will always be an exact match. Use combinations of the search fields to narrow your target. If a search fails, try deleting extra fields to broaden your search or enter a different spelling of the applicant name, and press the search button again. To view a permit application summary click on the log number for that application.
 
Field Entry Tips:
county: name of county in which waterbody is located e.g., "Mecosta"
log#: application sequence number e.g., "1245"
applicant name: applicant name submitted on permit application e.g., "Smith, John" or company name e.g., "Happy Plant Control, Inc."
waterbody: name of waterbody e.g., "Long Lake" or "Huron, Lake"
 
Status Definitions:
received: permit application received by office and logged into database
hold for information: incomplete permit application / information missing
awaiting review: administrative review complete, awaiting environmental review
assigned to reviewer: administrative review complete, environmental review initiated
issued: permit application issued
withdrawn: permit application withdrawn by applicant
denied: permit application denied
closed - incomplete application: incomplete permit application closed, due to expiration of permit review season

The information provided does not grant authority for chemical treatment. If and when a permit is issued, it is valid only for the permit applicant and the permitted chemical treatment is legal only after the permit applicant has received the permit.
